Setup Username, Email Address, First Name, Last Name, Password (twice), then added a Custom Capability (teacher) and entered an EOT Date (08/15/2011).
Clicked on the "Add User" button.
User was created, but no Custom Capability or EOT date (you can immediately tell now that the capabilities are listed on the User Listing page).
You can immediately "Edit" the user created to re-add the CC and Date.

Active Plugins that may affect this:
New User Email Setup
Role Manager
User Switching

Can you please walk me through quickly, exactly what we need to reproduce this?

For instance, were you on the admin page " user-new.php " when you created the user?
If so, what Role did you assign to this new User?
What other plugins do you have installed?

You have to immediately click "Edit" on the user and enter that information again and then save for it to stick.

Just thought I would report the bug.
